LEAD Case Manager

Who are you?

You are a caring professional who is dedicated to providing mental health and/or substance abuse services within your local community. You understand the importance of our LEAD (Let Everyone Advance with Dignity, formerly known as Law Enforcement Assisted Diversion) program which provides comprehensive services to adults who are experiencing behavioral health challenges, the support they need to achieve improved quality of life and reduce the tendency of a convicted criminal to reoffend.

Be a part of a program who's goal is to decrease the reliance on both policing and the legal system in order to respond to public order issues related to drug use and unmet behavioral health needs. Provide outreach, behavioral health treatment and recovery support, and linkage to a variety of community resources. Lived experience as a past consumer of mental health and/or substance abuse services is important for this role. Your story matters, you matter, and you can make a difference in helping others overcome barriers to recovery.

Under the guidance of the Criminal Justice Program Supervisor, you will establish a working relationship with participants in the community and engage in services. Provide a successful handoff to our case management team and other treatment providers to engage the participants in necessary wraparound services. Provide advocacy for participants within the criminal justice system, including but not limited to, court appearances. Record client progress, monitor relapse plan and maintain necessary records and files.

You will ensure adequate client care, provide consultative services and/or promote the coordination and development of mental health and/or substance abuse services within the community. Working in a team based environment is important to you. While working with the treatment team, you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client.

You have:
  • HSD/GED
  • A significant period of continuous recovery and awareness of the importance of recovery in living with a mental illness or addiction problem
  • A valid driver's license and the ability to maintain auto insurance, as the position requires transporting clients in your personal vehicle
  • The willingness to potentially travel as needed to other FRC locations that include: Kenton, St. Marys, and Lima
  • Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously.
  • CPR/First Aid Certification or willingness to obtain one
  • Willingness to submit background check and fingerprinting

Preferably:
  • Previous relevant experience working with individuals that have a primary mental health diagnosis and/or co-occurring disorders.
  • Knowledge and familiarity of available community resources
